Family Coordinator - Healthcare

About the role

We are seeking a full‑time Family Coordinator to manage newly authorized client cases from authorization through early service stabilization. You will act as the main liaison for families, guiding them through onboarding, service start‑up, and the transition to case management.

Key responsibilities

• Serve as the primary point of contact for families during service initiation.
• Guide families through onboarding steps, timelines, and expectations.
• Coordinate assessments, provider assignments, and service start dates.
• Ensure services are scheduled and providers are ready to begin.
• Track early service delivery and resolve issues affecting consistency.
• Partner with internal teams to maintain timelines and service readiness.
• Coordinate the transition of stable cases to Case Management.

Required profile

• Advanced English proficiency.
• Strong customer‑service skills for interacting with caregivers, patients, and insurers.
• Excellent organizational abilities to handle documentation and fast‑paced coordination.
• Clear, professional communication across multiple stakeholders.
• Ability to manage timelines, priorities, and follow‑through.
• Problem‑solving mindset with focus on execution.
• Attention to detail and accountability for outcomes.
• Proactive and creative approach.
