Remote Patient Monitoring (RPM) Care Navigator

About the role

The Remote Patient Monitoring Care Navigator supports patients enrolled in an RPM program by reviewing vital‑sign alerts, conducting outreach calls, and coordinating follow‑up with clinical teams. This full‑time remote position requires strong communication, organization, and attention to detail.

Key responsibilities

• Review and respond promptly to out‑of‑range blood pressure and heart rate alerts.
• Contact patients by phone to assess symptoms, confirm readings, and determine need for additional follow‑up.
• Escalate urgent or clinically concerning findings according to established protocols.
• Conduct monthly wellness check calls and document all interactions in the Amana platform and Athena EHR.
• Collaborate with practice operations managers, medical assistants, and other care team members to resolve patient questions.

Required profile

• High school diploma required; associate or bachelor’s degree in healthcare, public health, or related field preferred.
• Experience in patient outreach, care coordination, call‑center environment, or medical assisting.
• Strong verbal and written communication skills and ability to maintain HIPAA confidentiality.
