Remote Patient Monitoring (RPM) Care Navigator

About the role

The Remote Patient Monitoring (RPM) Care Navigator supports patients enrolled in a remote monitoring program by reviewing vital‑sign alerts, conducting outreach calls, and coordinating follow‑up with clinical teams. This full‑time remote position requires strong communication, organization, and attention to detail.

Key responsibilities

• Monitor blood pressure and heart rate alerts and respond promptly.
• Contact patients by phone to verify readings, assess symptoms, and determine need for further action.
• Escalate urgent or clinically concerning findings according to established protocols.
• Perform monthly wellness check‑ins with all enrolled patients.
• Document outreach, alerts, actions, and escalations in the Amana platform and Athena electronic health record.
• Collaborate with practice operations managers, medical assistants, and other care team members.
• Track unresolved issues and ensure appropriate follow‑through.
• Maintain patient confidentiality and comply with HIPAA.

Required profile

• High school diploma required; associate or bachelor’s degree in healthcare, public health, or related field preferred.
• Experience in patient outreach, care coordination, medical assisting, or call‑center environments.
• Strong verbal and written communication skills and ability to remain calm with patients experiencing health concerns.
• Excellent attention to detail and ability to manage multiple tasks efficiently.

Required skills

• Experience working with electronic health records (EHR) systems.

What we offer

• Salary range of $1,000–$1,200 per month, based on experience.
• Full‑time remote schedule: Monday‑Friday, 9:00 am–6:00 pm CST.
